Dodging is the only way you can avoid damage from Pyramid Head's attacks. Keep your distance and try to kite around him when possible. If he gets too close and attacks, time your dodges as he can still hit you if you dodge too early or too late.
While we cannot confirm if this actually does more damage, it sounds more realistic given how this monster has a giant metal triangle covering its head.
Though rather risky, if you're low on ammo, you can engage Pyramid Head in melee. His attacks do hurt, but they're also slow, giving you a few seconds after a well-timed dodge to sneak in a melee attack or two.
If you've studied his attack patterns enough and feel confident, you can try beating this boss purely in melee.
All firearms are generally more effective on an enemy like Pyramid Head. However, the Hunting Rifle outputs the most damage per shot and allows you to accurately hit Pyramid Head well outside his effective combat range.
Hunting Rifle Location
Pyramid Head with a Spear is a bit faster. His attacks are weaker, but quicker and farther-reaching.
The safest bet during this fight to to stay outside of armsreach of both Pyramid Heads and to constantly sprint-and-shoot, staying along the edges of the area.
Concentrate you attacks on the closest Pyramid Head. Though there are 2 of them, their health seems to be shared, so it'll be easier to just damage whoever is closer.
We recommend still going with the Hunting Rifle because of its accuracy and high damage. It worked best against Pyramid Head in the original and still holds true here.
