Solving the X-ray puzzle requires you to extract the code for the combination lock found across the hallway. James must explore the hospital to find the fourth radiograph in addition to the three already available in the X-ray room.

1. Go to the X-Ray Room on the Third Floor
2. Get the Pelvis Radiograph and the Memo on the Table
3. Get the Fourth, Moldy Radiograph
4. Get a Mold Remover
5. Rearrange The Radiographs to Get the Code
6. Enter the Code Into The Combination Lock

Go to the X-Ray Room on the Third Floor

First of all, you need to reach the X-ray room found on the third floor of Brookhaven Hospital. You will need to break the glass to access Exam Room 5 and then continue walking inside until you reach the X-ray Room.

Get the Pelvis Radiograph and the Memo on the Table

Before approaching the X-ray viewer, get the Pelvis Radiograph on the table. It is part of the puzzle solution so it's good to have already.

Beside the Radiograph is a Doctor's Memo revealing that there is a fourth radiograph that you need to find to complete the puzzle.

Get the Fourth, Moldy Radiograph

The fourth radiograph is found inside the Nurses' Station on the second floor of the hospital, withering inside a moldy bath tub. You will have to get the combination and access the room.

Get a Mold Remover

Once you've retrieved the fourth radiograph, you will notice that it is covered in molds. In this case, you must get the Mold Remover found on the insect-riddled trash chute in the first-floor pantry.

Once obtained, combine the moldy radiograph and the mold remover to reveal a clean radiograph!

Rearrange The Radiographs to Get the Code

Once the fourth, clean radiograph is at hand, go back to the X-ray room to solve the puzzle. You have to align the scratches on the radiograph that will reveal a code if done correctly!

To solve the puzzle, you must align markings on the radiograph and the viewer. The number of markings on the viewer will decrease as you increase the puzzle difficulty!

Light & Standard	Markings on the viewer for all four radiographs are present. It is just a matter of aligning the markings to match the viewer's and the radiograph's.
Hard	Markings on the viewer will only be available for the skull and lungs radiograph. You will have to experiment and align the scratches so the pelvis and hand radiographs perfectly bridge the first two radiographs.

The code obtained is the same for all difficulties: 4 - 37- 12. The presence of markings is the only difference across difficulty levels.

Enter the Code Into The Combination Lock

Once the code is obtained, enter the code into the combination lock. Three symbols appear to the left of the combination lock denoting the turn direction per number. As mentioned, the code is the same for all difficulty levels: 4 - 37- 12.

Opening the combination lock gives you access to the room where you can get the Filthy Bracelet.
