To get the Bliss ending, you need to drink some White Claudia before watching the tape in Room 312 of Lakeview Hotel. You can get this item by opening a box found in Brookhaven Hospital using a key from Pete's Bowl-o-Rama.

Get the Rusted Key from Pete's Bowl-o-Rama

Head over to Pete's Bowl-o-Rama in Western South Vale. Find the safe behind the counter and enter the code 1887. Pick up the Rusted Key found inside the safe.

This should be done before going to Jacks Inn since Pete's Bowl-o-Rama will become inaccessible later on. The safe inside the bowling alley will only show up on a New Game Plus playthrough.

Get the Small Chest in Brookhaven Hospital

Go to the garden in Brookhaven Hospital and look for the gazebo. Inside, you can pick up a small chest that doesn't appear on a normal playthrough. Use the Rusted Key on the chest and to collect the bottle of White Claudia.

When you get to Room 312 in Lakeview Hotel, drink the White Claudia before watching the tape. This is where the story will end, skipping the final boss encounter.

What is the Bliss Ending?

James is Stuck in a Fantasy

When the camera cuts back to an empty chair and a shut off TV in Room 312, it is implied that James has somehow returned to the time he and Mary were on vacation in Silent Hill.

However, White Claudia has appeared in other Silent Hill games and is known to be a hallucinogenic substance. The ending shows that James would rather drink the White Claudia and live a lie rather than face the truth.
