Focus on Maria

To get the Maria ending, you must do your best to focus all of James' attention on Maria. Elongate the time spent with her by fully exploring areas as she follows James around, make sure she is not hurt in any way, and constantly check on her. The other requirements needed are similar to the Leave ending, with the only exception being looking out for Maria.

Always Pay Attention to Maria

Whenever James is with Maria, make sure to spend as much time with her as possible and humor her teasing. The best area you could do this in, is after meeting her in Western South Vale, where Maria stays with James the longest before separating in Brookhaven Hospital. You can also choose to interact with her in Room 108 of Jacks Inn after she raises a very familiar outfit, for extra brownie points.

Alongside bonding with her, always ensure that Maria never sustains heavy damage from any enemies you encounter. As long as James keeps being protective over Maria and shows concern over her, you will be well on your way toward the Maria ending.

Check on Maria When You Leave Her

Make sure to check back on Maria in Brookhaven Hospital after you leave her in Room C5, and revisit her body when she is found dead inside the Labyrinth. These are two instances that you must come back to check on her more than once to really drive home James' concern for her.

Keep Health Up

Just like in the Leave ending, you must make sure that James pays attention to his health by using healing items as soon as he sustains damage. This will prevent you from possibly getting the In Water ending.

Do Not Look at Mary's Photo

Since this ending requires James to turn his attention to Maria, make sure that is reflected by paying less mind to Mary and never checking her photo or letter in James' inventory.

While going through the last hallway in the Lakeview Hotel Otherworld, immediately head through the door instead of listening to the entirety of Mary's monologue as she confronts her mortality and loneliness.

After Angela leaves her knife with James and he keeps it in his inventory, never inspect it. This is one of the key requirements needed to get the In Water ending, so it is best to avoid it.

What is the Maria Ending?

James Leaves Silent Hill With Maria

The Maria ending allows James to leave Silent Hill, much like in the Leave ending, though this time he leaves with Maria in tow. Narratively, this ending shows that James has learned nothing; He chooses Maria as a means to escape from his guilt instead of facing the demons that eat away at his conscience, dooming himself to repeat history.

This is especially seen when he tells Maria to "Do something about that cough" after showing signs of a sickness that mirrors his late wife, continuing the vicious cycle of helplessness, despair, and hatred.
