Keep James' Health Low in the Game
To achieve the In Water ending, players need to keep James' health low for most of the game, denoted by a bloody red border on the screen. This symbolizes his lack of regard for his being, as with Mary gone, he has lost his drive in life.
Examining Angela's Knife repeatedly will also lead towards the In Water ending, as the knife is symbolic of Angela's dejected views about her own life, which James comes to relate to during the course of the game.

Keep Health Low
When James suffers damage, do not immediately heal him. Instead, you may let his health drop to death's door and remain in this critical state for as long as possible before regaining health.
This shows that James' regard for his wellbeing is at an all-time low, which is an important criteria for the In Water ending.
Constantly Inspect Angela's Knife
After Angela hands over her knife to James, make sure to inspect it regularly in your inventory. Doing so gives the impression that James is thinking about and resonating with Angela's grim outlook on her own situation.

Much like in the Leave ending players must spend as little time as possible with Maria, and ignore her advances while she travels with James. Don't let her die to monsters, but also don't go out of the way to protect her.

During parts of the game where you leave Maria in a room at Brookhaven Hospital, or after seeing her dead inside a cell at the Labyrinth, never check back on her after exiting the room.

What is the In Water Ending?
James Drives Into Toluca Lake
In Water is the ending where James concludes that his life is meaningless without Mary, and decides that the best thing to do now is to join her in the afterlife. With this in mind, he gets into his car again, and speaks his mind out loud.
The screen cuts to black, and the sound of a car crashing through barriers, followed by a loud splash, is heard. Mary's Letter then drifts to the bottom of Toluca Lake, along with her husband.
Required for the Stillness Ending
In Water is a prerequisite for one of the new endings added to the Silent Hill 2 Remake, called Stillness. It is an alternate version of the In Water ending, with some changes in how it plays out.

Anonymous about 1 yearI checked on Maria multiple times in the hospital and even got the dialogue for checking on her, i checked the knife like 2-3 max, i did spend like an hour on critical in the hotel tho cause i just couldn’t find any heals so maybe the requirement is mostly related to critical health

My health definitely wasn't low for "most of the game" (at least having the screen be red, maybe bloodstain on James back but I doubt it) and I got this ending without trying on my first playthrough. So yeah, the information in this article isn't accurate..
