Unlocking the Rebirth ending is relatively simple, as it requires you to finish an initial playthrough of the game, then replay it again in New Game Plus. During this playthrough, you will need to collect the White Chrism, Obsidian Goblet, and the books Lost Memories and Crimson Ceremony, then proceed to the end of the story without needing to worry about the requirements on other endings.

Start New Game Plus

Complete an initial playthrough of the game. Afterward, start a New Game Plus save file and progress through the game however you prefer, though keep in mind to collect all four ritual items before proceeding into the Lakeview Hotel Otherworld. Note that there is no other way to get this ending in a normal playhtrough as the items only appear during New Game Plus.

Obtain All Ritual Items

Collect the Crimson Ceremony, White Chrism, Obsidian Goblet, and Lost Memories in their respective lcoations around Silent Hill and make sure to have all of them in your inventory before entering Room 312 of Lakeview Hotel. Be careful not to miss a single item, as you can no longer revisit past areas, and the ending will not trigger if the items are incomplete.

What is the Rebirth Ending?

James Attempts to Resurrect Mary

The scene opens with James sailing through the fog of Toluca Lake, his boat carrying what appears to be Mary's body wrapped in white cloth. Driven by despair and armed with all the ritual items from the abandoned town, James intends to resurrect her by appealing to the Old Gods of Silent Hill.

As he rows toward an island to presumably conduct the ritual, the outcome—whether he restores Mary, damns them both, or nothing at all—remains unknown.
