The consequence of these choices will be observable in the Secondary quest Return to Crookbag Bog.
If you chose to free the spirit it will save the oprhans from the Crones, but the spirit will also attack the village of Downwarren, killing many.
The Crones, angry that the kids were taken under Anna's care and will turn her into a water hag.
After the curse is lifted, Anna reveals that the curse was tied to her life, lifting the curse kills her shortly after. The baron then commits suicide over the loss of his wife.
Killing the spirit saves the town of Downwarren, but the orphans get taken by the Crones, heavily implied to be eaten by them.
Anna from the shock of losing the orphans, looses her mind. The baron then takes her to find a cure far far away.
Gameplay-wise, it is best to kill the spirit. The battle is not overly difficult, but if you do still have trouble, you can choose to trick it in order to kill the spirit while avoiding combat.
You can also finish the quest Return to Crookbag Bog quicker, avoiding extra objectives which are required if you choose to free the spirit.
If you first say you will release the spirit but then trick it, you will kill it while avoiding a fight.
The spirit will cover itself in roots. Hit the roots until the heart is vulnerable. Endrega Workers will also spawn while you try to do this.
