Speak with Malena in Gerudo Town while she is speaking with one of the guards to start this side quest.
There are two things you need before you can start Medicinal Molduga side quest:

Find Malena while she is speaking with one of the guards and she will ask you to help her get some Molduga guts for a medicine. |

There are 4 Moldugas in Gerudo Region and the nearest one is just west of Gerudo Town and southeast of Toruma Dunes. Defeat one of them to obtain Molduga guts. Note: If you have already defeated a Molduga before, you can skip the killing part and give the guts to Malena right after you accept the side quest. |
| 3 | Return to Malena | Speak with Malena after obtaining the guts and give it to her and she will thank you with a Gold Rupee. |
Malena will give you a Gold Rupee as thanks for giving her some Molduga guts.
