When confronting the Dragonlord close to the end of the journey, the villain will make the Hero an offer: Join forces and rule half the world alongside the Dragonlord!
Accepting the Dragonlord's offer causes the screen to fade to black, returning you to the most recent checkpoint. This marks it as an optional bad ending.

The only path to Dragon Quest 1's conclusion is to defeat the Dragonlord! Upon defeating this final boss, peace will return to Alefgard.
After the ending plays out, you can return to the last location before fighting the Dragonlord to address any remaining side quests or collectibles, such as Mini Medals.
Final Boss Guide: How to Beat the Dragonlord

Though the story of Dragon Quest 1 must end with the Dragonlord's defeat, you can unlock a special variation of the ending by bringing Princess Gwaelin all the way to the final battle. This means carrying her almost everywhere after her rescue, and refraining from returning her to Tantegel for nearly the entire game.
While it can be quite the task to carry the princess all over the land, your reward is a large amount of additional dialog and cutscenes as the princess comments along the journey, incuding the ending! Additionally, the Princess' Necklace accessory will be upgraded to the Princess' Pledge if you beat the game with her at your side.
How to Rescue Princess Gwaelin

Unlike in the original game, rescuing Princess Gwaelin is now a required part of the story in the HD-2D Remake. This means the alternate ending where the Hero ventures out alone is no longer possible.

Defeating the Dragonlord with Princess Gwaelin directly connects the events of Dragon Quest 1 and Dragon Quest 2. This is because The Princess' Pledge is present in Dragon Quest 2, and the way to upgrade the Princess' Necklace to the Princess' Pledge is to beat the game with her alongside the Hero.
This cements the detail that the Hero of Dragon Quest 1 defeated the Dragonlord in Princess Gwaelin's presence, making it the canonical conclusion. The additional scenes added in Dragon Quest 1 and 2 HD-2D Remake also connect both games more strongly to Dragon Quest 3, which predates 1 and 2.

There are two trophies related to the player's selected ending, with the true ending trophy unlocked by defeating the Dragonlord, and the bad ending trophy obtained by siding with the Dragonlord.
