Here are the base recipes for Fairy Tonics. You only need a single Fairy to get started, and then you can add Creatures, Gemstones or Monster Parts to increase the efficiency.
Note: The creatures mentioned here are all the creatures that don't count as "Food ingredients", such as bugs, lizards and frogs. Basically, all the Creatures that come after Fairies when you sort your inventory are safe to use, and the ones before are not!

The order of the ingredients doesn't matter, as you dump everything into the cooking pot at once!
If you add normal cooking ingredients, it will result in a different dish with +10 Heart recovery instead of a Fairy Tonic. For example:

The only real effect the Fairy Tonic has is to heal a lot of hearts. Unlike the Fairies used for ingredients, it cannot activate on its own to revivie you if you die.
However, while Fairies "only" heal you 5 hearts after reviving you, a basic Fairy Tonic heals 12 hearts, so pausing just before you die and drinking one is highly recommended. Of course, keeping both on your will cover all bases!
The easiest way to come by Elixirs, by far, is to make them yourself. The other elixirs can also be traded from Beedle at random, but the only way to get Fairy Tonics is to make them yourself!
Go to the Great Fairy Springs and sneak up on the fairies there to get started.
