To unlock the Mahjong mini-game, you will need to complete all four "That Narumi..." side quests firsts. While you can start the first quest during, Episode 2, you will only be able to unlock Mahjong as early as Episode 4, since that's when the fourth case file is accessible.
As a note, you don't need to complete the previous side quest to unlock the next one, since it's non-linear, meaning you can complete them in any order that you wish.
List of All Case Files (Side Quests)

Once you've completed all four "That Narumi..." case files, you'll be able to play Mahjong at the Narumi Detective Agency. Do note that this mini-game uses the Japanese Mahjong ruleset.

After selecting to play Mahjong, you will need to pick who your opponents will be. If you want, you can choose to play with Narumi, then you can pick which demons you want to play with.

Unless you've played Japanese Mahjong before, it may be hard to understand what's going on. It's highly recommended to read the rules in the Detective Journal first so that you know what you're doing and can enjoy the game.

Whenever you play Mahjong, it's recommended to play against demons that have 3-Stars beside their name. These are the hardest opponents you can face, but they will give you a lot more money whenever you beat them.
Money Farming: How to Earn Yen Fast

Before you start a game, it's recommended to make a save first. This way, if ever you lose, you can reload to right before you started playing.

Mahjong is a playable mini-game at the Narumi Detective Agency. Whenever you beat Narumi or allied demons at this game, you will earn some money that you can use for your adventure.
You can skip this mini-game if you wish since it's not required for the story. However, it's recommended to at least unlock it since there is an achievement for completing every case file.
