Completing the Brothers in Arms quests does not affect the progression of the main story in the game. Completing the Brothers in Arms quests are completely optional but, completing them will only benefit the player. The more Brothers In Arms quests you complete the easier it is to complete the Main Quest: The Battle of Kaer Morhen.
If you care about unlocking character endings and getting additional items and monetary rewards then it is worth it to complete the Brothers In Arms quests.
In Velen, you can recruit two of Geralt's friends to fight with you at Kaer Morhen. You need to complete their respective side quests and convince them to go to Kaer Morhen during your conversations with them.
Having successfully recruited Keira Metz will give you a Herbalist to buy and sell from just before the battle.
Having successfully recruited Letho will give you another partner to be with when you go out of Kaer Morhen to battle the Wild Hunt.
In Novigrad, there are some characters who you need to do their respective side quests and very specific dialogue choices in order to get help from them. There are also some character where you do not need to do anything in order to recruit them.
Having successfully recruited Zoltan, he will be an Amateur tier blacksmith that you can buy, sell, craft, or repair from just before the battle and he will ready multiple explosive barrels that you can use during the battle inside the Kaer Morhen fortress.
Having successfully recruited Roche and Ves will make them plant spike traps outside the Kaer Morhen fortress so you have to deal with less enemies when closing the Wild Hunt's portals with Lambert and possibly Letho.
When you talk to Dijkstra, there is no way for him to agree on going to Kaer Morhen with you to defend Ciri. But if you make the right dialogue choices during the Main Quest: Count Reuven's Treasure, he will reward you with a 1000 Crowns and sell you a variety of items.
Triss will come to defend Kaer Morhen with you regardless if you finish her respective side quests.
Having successfully recruited Hjalmar with or without Vigi and/or Folan with give you more allies to fight the Wild Hunt with during the battle inside the Kaer Morhen fortress.
Having successfully recruited Ermion will make him open cracks which release explosive gas inside the Kaer Morhen fortress. You can use these by igniting them during the battle inside the Kaer Morhen fortress.
Cerys will always refuse to come to defend Kaer Morhen with you but, she will refer you to recruiting her best warrior instead, Hjalmar.
Crach will also always refuse to come with you to defend Kaer Morhen. But, if you have helped his children during their respective side quests, he will reward you with the Relic Steel Sword Winter's Blade.
When talking to Emhyr, it is impossible to get help from him because Geralt heavily disagrees with Ehmyr's conditions in order for him to send aid.
