The Miraculous Guide to Gwent Book can be obtained by playing and beating Aldert Geert at Gwent during the game's prologue.
However, if you didn't play him, then you can purchase the book from a shop in Gildorf, which can be found right behind the Notice Board near the St. Gregory's bridge fast travel point.

The Miraculous Guide to Gwent is a book that shows how many cards you must obtain before completing Gwent Quest: Collect 'Em All. These include purchasable cards, cards obtained from Gwent Quests, as well as randomly obtained cards from NPCs all over the different regions in the game.
