During the main mission Scenes From a Marriage, Geralt will come across the choice to take or leave a rose. While this decision mainly affects the character Iris, Geralt can also gain some useful information should he take the rose.
Scenes From a Marriage Walkthrough, and Best Rose Choice
Taking the rose results in Iris being free, removing her from all the suffering that she's endured. You'll also gain useful information about Gaunter should you ever choose to stand against him.
Although Iris has been suffering for a long time, she also doesn't want to die. Since the rose binds her to the world, the rose prevents her from facing death, or the void, as she calls it.
If you want to respect her wish to not die and face oblivion, the choice to leave the rose is always an option.
