Talk to Otus by the armory located southwest of Pyrgos Village, and he will warn you about the Minyades around the area going hostile against the locals. Agree to help resolve the issue to start the quest.

1. Find the Reason for the Minyade Attacks

Titan Quest 2 Minyade Location

Walk northeast from Pyrgos Village after descending to the Flooded Farmlands and head toward the Shrieking Ruins in Southern Flooded Farmlands. There, you'll find a band of Ichthians disturbing the local Minyade population by making so much noise. Defeat all of the Ichthians in battle to stop the Minyade frenzy.

2. Talk to the Minyade Leader

After defeating all of the Ichthians, speak to the Minyade leader posted by the staircase leading outside of the ruins. They will thank you for your service and ask to have an item delivered to Otus.

3. Talk to Otus

Titan Quest 2 Return to Otus
Return to Pyrgos Village via its Rebirth Waypoint and return to Otus by the armory southwest of the village. Present the Minyade leader's token of friendship to him to conclude the quest after your conversation!
