Speak to Lochagos Rizon in Pyrgos Village and choose the Do you need anything done?
option. This will prompt him to speak about defeating four Ichthian commanders within the Flooded Farmlands.
You may begin your hunt with any Commander as you please, though we recommend starting from the South and working your way toward the North Commander as their levels also increase.

The first Commander you can encounter is located in the Southern Flooded Farmlands, within an encampment called the Foggy Hideout. There, you'll face the Ichthian-Scales-Like-Shell, a level 9 Elite enemy.
This enemy heavily relies on applying Poison DoT effects through both close-range and ranged attacks. Be sure to dodge its attacks or have good Poison resistance in case you get hit. Pick up the Commander Medallion that it drops after defeating it!

The next Commander can be found in the Western Flooded Farmlands, within an area known as the Butcher's Fort. Here, you'll encounter the Ichthian-Claws-of-Crimson, a level 11 Elite enemy that primarily deals Cold damage.
This Commander tends to remain stationary but is capable of unleashing relentless ranged and AoE attacks that apply Cold debuffs. It's best to eliminate the smaller enemies around the fort first to clear the field, then concentrate your efforts on taking down the Elite. Don’t forget to collect the Commander Medallion it drops upon defeat!

Next, head east toward the lower region of the Northern Flooded Farmlands to locate the third Commander, the level 14 Ichthian-Bile-that-Burns, positioned within the Roadside Encampment.
This Elite enemy closely resembles the first Commander, specializing in Poison DoT effects via ranged and AoE attacks. Stick to the same strategy you used against the Southern Commander, and as long as you're on par with its level, you should have little trouble. Be sure to collect the Commander Medallion it drops afterward.

Lastly, head to the northwestern region of the Northern Flooded Farmlands and pass through the Secluded Beach in order to reach the Clifftop Outpost. There, you'll meet the Ichthian-Fist-That-Breaks, a level 15 Elite enemy.
Unlike the previous three Commanders, this one doesn’t inflict status effects but makes up for it with powerful swings and wide AoE attacks that can deal heavy damage if you're not careful. Thankfully, its slow wind-ups give you enough time to dodge, reposition, and land attacks between its moves.
Ranged weapons work best here, but as long as you balance dodging and striking effectively, you’ll get through the fight with little trouble. Defeat it, claim the final Commander Medallion it drops, and return to Pyrgos Village.

Teleport back to Pyrgos Village via the Rebirth Waypoint and return to Lochagos Rizon. Speak to him and turn over all of the Commander Medallions you've collected in order to complete the quest.
