This choice can affect the Secondary Quest There Can Only Be One where Geralt must prove his compassion.
If you choose to spare the Shaelmaar, the deed will be count as Geralt proving his compassion. If you choose to kill the Shaelmaar, then it will not affect the quest stated above.
If you chose to use Axii to calm down the horse and get the clue on the Unicorn's horn, a group of Courtiers will approach you after getting the clue. If you cast Axii on the Courtiers, you will avoid a 3 on 1 fist fight with them and move on. If you choose either the 2nd or 3rd option, it will end with a 3 on 1 fist fight with the 3 Courtiers.
After getting the Unicorn Horn and the Golden Fish, Geralt and Anna Henrietta will have to solve a riddle to figure out where Milton is. The answer to the riddle is "The greenhouse". But even if you choose wrong, Anna will get the right answer anyway. Getting it right will count towards the virtue of wisdom in the quest There Can Be Only One.
In this part of the quest, you can either cast Axii on the horse to immediately calm it down, or pick an apple in the large basket to your right near the people having a picnic to lure it to you.
The Golden Fish is located under the southernmost fishing boat nearest to the shore. If you get the contents of 2 or more fish that are not the Golden Fish, a cutscene where Geralt chases a boat will play when you get near the Golden Fish.
After gathering all the clues to the location of Milton's hideout, you are given multiple choices to answer which one is the correct hideout.
The correct answer will be the option It's a greenhouse!. Either way, if you pick any of the other options, Anrietta will still figure out the right answer to all of the clues.
