You can encounter this fight during the Hearts of Stone Main Quest: Scenes From a Marriage. The best way to deal with The Wraith from the Painting is to constantly protect yourself with the Quen Sign during its relentless attacks and immediately destroy the green portal every time it spawns one.
In this fight the Yrden Sign trap hardly reduces the speed of the wraiths movement or attacks so it is not recommended to waste your stamina in casting this Sign. It is better to protect yourself with the Quen Sign and attack the wraith.
When the wraith's HP falls to a certain point it will fly up to the ceiling and spawn a green portal. When this happens use your full arsenal to hit the portal and close it as soon as possible. If the wraith is able to enter the portal, it will come back out of it with bonus HP.
You should use your Silver Sword and coat it with Specter oil to deal bonus damage to them. Prepare bombs that deal damage like the Grapeshot Bomb and Dancing Star Bomb in order to close the portal from a distance.
