The best equipment in The Witcher 3 are primarily Witcher gear sets that can be found throughout the game by completing scavenger hunts. Witcher gear sets provide unique effects that can increase Geralt's signs, adrenaline gain, and toxicity endurance, among all others.
For example, the Ursine Armor Set grants improved adrenaline gain and excellent defense, catering to those who wish to play a tanky playstyle. On the other hand, the Wolven Armor Set provides an all around boost for players who want to engage in most of the game's combat mechanics.

The best equipment available in White Orchard are the upgraded version of your starting armor, the Warrior's Leather Jacket, which can be upgraded after you finish the Side Quest: Twisted Firestarter. On the other hand, you can get the Temerian Armor Set by purchasing it from the merchant you saved from the Griffin.
The best swords you can have in White Orchard are both the Viper School Serpentine swords. You can get these by completing the Scavenger Hunt you receive after looting the Bandit Camp near the Ransacked Village.

The best equipment in Velen and Novigrad would be the Griffin Armor Set and the Feline Armor Set. You need to complete their respective Scavenger Hunts to get either of the armor sets.
The Griffin Armor Set is the best equipment that you can use if you want to focus on using signs during combat, while the Feline Armor Set focuses on empowering light attacks in combat.

Once you reach Skellige, the Ursine Armor Set will now become the best equipment available. Just like the other Witcher Gears, you need to complete its respective Scavenger Hunt in order to get the diagrams for the armor set.
The Ursine Armor Set gives you a large adrenaline point gain bonus once you complete the full set. This armor set is also very defensive, giving you a lot of protection against enemies.

Once you are able to travel to Kaer Morhen, the best equipment available now becomes the Wolven Armor Set. This armor is considered as an all-rounder set and gives you bonuses on all of Geralt's skills.
It is also important to know that when you reach Kaer Morhen, you should have already completed the Side Quests: Of Swords and Dumplings and Master Armorers. Completing these quests will allow you to craft the Mastercrafted versions of the Witcher Gears from Hattori and Yoana.

The best equipment in the game can only be found in Toussaint. In this region, a Grandmaster Smith can be found in southern Beauclair. This NPC can upgrade mastercrafted armor sets to grandmaster tier, making them more powerful than before.
For the best weapons, the Aerondight is the best silver sword due to its capability to increase damage with each kill. Additionally, this sword gets 100% crit chance once fully charged. For the best steel sword, the Toussaint Knight's Steel Sword takes the crown for its unrivaled damage output and armor piercing.
