You can encounter this fight during the Main Quest: On Thin Ice. You will have to battle him twice, one time using Ciri and another to finish him off as Geralt.
The best way to deal with Caranthir as Ciri is to always close the gap between the two of you using Ciri's skills and continually attack him while you are close to you.
The best way to deal with Caranthir as Geralt is almost the same with using Ciri. You need to close the gap between the two of you as fast as possible while protecting yourself with the Quen Sign. When his health reaches a certain point he will start summoning Ice Elementals. You can either deal with the Ice Elementals first to leave Caranthir alone, or you can use the Ice Elementals as shields from Caranthir's attacks in order to get close to him.
You should use your Silver Sword and coat it with Elementa oil to deal bonus damage to them. Prepare some Dimeritium Bombs if prefer to deal with the Ice Elementals before going back to Caranthir.
