Doom Slayer
You can get the Doom Slayer codex entry on top of the plateau behind one of the demon portal objectives in Village of Khalim.
Deag Loric
Dead Loric's codex page can be found on the helicarrier located at the lower sections of the city in Chapter 5. This can be found near the last area of that helicarrier after beating the horde of enemies.
Kreed Maykr
Kreed Maykr's codex from a small elevated section found on the rightmost area of the battlefield in Chapter 6.
King Novik
You can get King Novik's codex entry near the area leading up to the boss fight in Chapter 9. Head lower down some platforms to reach this Codex.
Prince Ahzrak
The Prince Ahzrak codex page in the area that will require you destroy 2 Hell Cannons in the Chapter 10. You can access this area using a hidden purple key.
Serrat
Located just before the elevator that leads to the Old One release mechanism area in Chapter 14.
The Witch
At the beginning of the mission in Chapter 16, run towards the right onto the next platform. Look to your left and you'll see an interactable gear. Throw your shield to open the gate containing the Codex entry and a Gold Cache.
Commander Thira
The Commander Thira codex entry can be found near an acid pile found near the end of the chapter after destroying the third green organ in Chapter 18.
Hell Priests Chapter 19 Hell Priests Codex mode:scale
In the final area before Chapter 19 ends, go off the beaten path and you should be able to find the Hell Priests Codex.
Commander Valen
In Chapter 20, after landing using the Serrat, the codex can be found behind you.
Marok
In Chapter 22, clear the demons in the starting area then head up the little cave up a hill near where you initially dropped off. Climb up the hill to find the Marok Codex, just a little past the first Life Sigil you find.

Character codices are obtained as collectibles throughout the whole campaign. These character entries provide deeper lore and information regarding the main characters in the game.

All Location Codex

✔️ Location How to Get
Village of Khalim
Towards the path leading to the final objective of Chapter 1, you'll find a Wall scramble you can climb up to your left. There, you'll find the Village of Khalim Codex page.
Hebeth
Jump over the metal boxes towards the left instead of proceeding to the objective.
Sentinel Barracks
Found at the left side of the Sentinel Shrine beside the wooden barrier before the Vagary Champion. Remember to also pickup the gold trail near this codex entry.
Holy City of Aratum
It can be found in the secret area that becomes accessible shortly after obtaining the Super Shotgun.
Siege Part 2
After getting the Purple Key, drop down into the courtyard area to find a door with a purple lock on the left end of the wall. Go inside and defeat the monsters to obtain the Siege Part 2 Codex.
Abyssal Forest
Obtained by going to a secret area, accessible by using a shield charge on the wall with a blue seal.
Ancestral Forge
Found at the end of the map near the upper west enemy camp. Remember to also pick up the gold trail near this codex entry.
The Forsaken Plains
From the rightmost sentinel shrine of the map, head right until you reach the Atlan hand where the codex is located.
Sentinel Command Station
Right after leaving the vents at the start of the chapter, continue straight until you find an opening on the wall to your right. You'll find the codex in this hole in the wall.
Spire of Nerathul
At the edge of the cliff where you can look up and see a Green Statue, look down off the side of the cliff to see an area you can fall to, containing the Spire of Nerathul Codex and Gold Trail 2 (x5).
City of Ry'uul
Access this secret area below the area where you first encounter an Acolyte. Go down a platform and follow the trail of gold until you reach the City of Ry'uul Codex.
Temple of Lomarith
As you swim through the waterlogged sunken ship, you'll come across the Codex entry floating in the middle of the pathway.
Harbor of Souls
At the edge of the cliff where you can look up and see a Green Statue, look down off the side of the cliff to see an area you can fall to, containing the Harbor of Souls Codex and Gold Trail 2 (x5).
Resurrection
Can be found in the same room as the Cosmic Baron Toy. This is obtained during the second objective.

Location codices can be found in hidden areas or as collectibles throughout the campaign. Most locations have a codex page tied to them, so make sure to search for as many as you can go through the campaign.

Weapon How to Get
Combat Shotgun
The Combat Shotgun is equipped at the start of the game.
Shredder
You'll acquire the Shredder in Chapter 1 during the Cover the Escape objective where you're tasked to destroy four demon portals.
Power Gauntlet
Acquired in Chapter 1 during your task of clearing the village Cover the Escape! objective. Progress the story as is and you'll eventually receive the Gauntlet.
Shield Saw
Acquired at the start of Chapter 2.
Accelerator
Acquired in the Research Lab of Chapter 2 during the Defend Hebeth objective.
Pulverizer
Obtained in Chapter 4 in the starting area.
Impaler
Acquired in Chapter 4 after going down the hole in the barracks.
Cycler
Acquired inside the third Hell Carrier in Chapter 5 during the Break the Cathedral Forcefield objective.
Super Shotgun
Acquired after your first encounter with Enforcers in Chapter 5.
Flail
acquired during Chapter 6 before you head on to the main battlefield.
Reaver Chainshot
Acquired in Chapter 6, during the objective where you must kill cultists to destroy the Gore Portal.
Grenade Launcher
Acquired in Chapter 8, after you descend the evelator during your objective of reaching the mine entrance.
Rocket Launcher
Acquired in the first area of Chapter 9.
Ravager
Acquired in Chapter 12 near a console
Ballistic Force Crossbow
Obtained in Chapter 14 after you complete the task where you must deactivate three Stasis Emitters.
Dreadmace
Acquired in Chapter 15 by following the quest marker during your objective to activate the portal.

Similar to enemies, Codex pages for weapons are unlocked after obtaining the weapons themselves. You'll unlock all the weapons as you progress further in the game as part of the challenges you'll encounter!
