Pandemonium runs almost on the same difficulty as Nightmare with a few extra considerations that will make the game a bit more challenging.
Life Sigils will no longer revive you on the spot, but will allow you to restart the Chapter upon death. There is a set amount of Life Sigils you can encounter throughout the campaign, forcing you to budget them on every level.

On lower difficulties, you could go back to a previous level and collect a Life Sigil you had not collected before to ensure your survival. However, on Pandemonium and Ultra-Nightmare, players will no longer be able to replay a level that's already been finished.

Much like Pandemonium, Ultra-Nightmare is just the same difficulty but with one crucial difference. You only get 1 life.
Life Sigils only act as health pickups on this difficulty. Once you die, your entire campaign is reset, including upgrades purchased, collectibles found, and challenges completed.

| Parry Window | Set to Medium by default. This will ensure a slightly smaller window of opportunity for parrying Hell Surge projectiles and attacks. |
| Enhanced Targeting | Set to Off by default. This will require more precise aiming when shooting at enemies. |
| Empowered Attacks | Set to Off by default. Enemies are no longer interrupted by the Slayer's attacks. |
| Damage to Player | Set to 250% by default. Enemy attacks will hurt more. This will require more careful movements and more consistent blocking and parrying. |
| Damage to Demons | Set to 100% by default. Your guns will have the same effectiveness on enemies as the other difficulties. |
| Enemy Aggression | Set to Most by default. Enemies will be more aggressive and will attack relentlessly. |
| Daze Duration | Set to 75% by default. Stunned enemies will recover faster forcing players to act faster to gain the advantage. |
| Enemy Projectile Speed | Set to Medium by default. Paired with the Parry Window, this makes it more challenging for players to dodge and parry ranged attacks.+ |
| Resource Values | Set to 2 by default. Ammo, Health, and Shield Pickups will have lower values. |

Survival should be your main concern on Pandemonium and Ultra-Nightmare. Don't rush in enemy groups that could easily overpower you and flank you from different sides. Combat is more or less the same as the lower difficulties. Attack patterns do not change, they just become more frequent.
Slowing down and pacing yourself leaves more room for you to plan your attacks and less room for mistakes to happen due to panic.
Focus on acquiring as much resources like Gold, Rubies, and Wraithstones as you can. You cannot come back and replay a level once you've completed it, making those necessary resources no longer available to you.
Some Mission Challenges will also reward resources. It is highly recommended to complete mission challenges to gather the total amount of resources necessary to fully upgrade all your weapons, so might as well attempt to complete as many as you can on your run.

| 16 - The Kar'Thul Marshes | ・Kill 20 or more demons with a single BFC Shot Reward: 100 Gold |
| 17 - Temple of Lomarith | ・Collect all Rubies and Wraithstones Reward: 100 Gold |
| 18 - Belly of the Beast | ・Complete the optional encounter in the stomach. Reward: 100 Gold |
| 20 - Resurrection | ・Destroy 3 Artillery Cannons ・Chase and destroy the Hell fighter ship. Reward: 100 Gold |
| 22 - Reckoning | ・Kill a large number of demons within 10 seconds. Reward: 100 Gold |
Upgrades do not carry over from lower difficulties, meaning you'll always start completely unupgraded at the beginning of every Pandemonium and Ultra-Nightmare run. We recommend upgrading as much as you can before finishing a chapter. This ensures that you have the most optimal weapons readily available to you as you progress.

Beating the game on the two highest difficulty rewards you with the in-game exclusive milestones, Pandemonium Complete and Ultra-Nightmare Complete. You will have to complete the game twice on differing difficulties to acquire both milestones.
