The Two-Eye Reef is an island that you can visit in the Great Sea. You will have to journey to this island during your Triforce Shard quest.

Eye Reefs Become Hostile Later in Game

After visiting the Forsaken Fortress the second time, the ship cannons on Eye Reef Islands will activate and will shoot at you when you're passing by.

Use bombs to destroy the island cannons and sink the ships inside the reef. After sinking all the ships, a chest containing a special chart will appear on the island.
