| 1 | Reach the Surface Your story begins in the Collezolfo Sulfur Mine. After a hard day of work, you and your friend, Gaetano, head back up to the surface to get a meal. |

| 2 | Sneak into the Storehouse Without Getting Detected You and Gaetano arrive too late, and can no longer be given food. Gaetano tells you to sneak into the storehouse to grab some bread. As you rest back in your bed, you are suddenly called back to work. |

| 1 | Go to San Celeste Don Torrisi and Don Galante have come up with a plan to counterfeit money as a means to counter Don Spadaro's growing influence. For the plan to work, they would need Giuseppe Palminteri, a forger. However, he is in jail and must be freed first. |

| 1 | Find a Way to Join the Race Leo is eyeing to join a big race coming up. With a few connections, he managed to obtain a Carozella, an exceptional car. While getting the car fixed up, there is still the issue of getting a spot in the race. Leo comes up with a plan to blackmail Marquis Siragusa, the event's organizer. |

| 2 | Ask Aroud Collezolfo Townsfolk point you towards Lorenzetti's office, but you find that it already looks abandoned. A worker will point out that Lorenzetti has been taken to Fontanella's office in the nearby factory. |

| 3 | Infiltrate the Factory Sneak through the armed men within the factory and make your way to the foreman's office. You will find Lorenzetti already dying. Spadaros got to him first, and you just fell into their trap. |

| 2 | Go to the Sniper's Tower As the festivities continue, gunfire is heard and people get shot. The sniper, Il Merlo, is up on the nearby tower. Take cover and slowly make your way towards the tower. |

| 3 | Defeat Il Merlo Il Merlo attempts to flee through the town's buildings, but you manage to stay right behind him. After a car chase, Il Merlo crashes along the way, where you duel him for the last time. |

Don Galante volunteers to be a neutral party and offers to host the meetup. The talk did not go well, and Fontanella tells Don Torrisi of his deal with Don Spadaro. They will effectively take Don Torrisi's land, and the meeting escalates further when armed men suddenly surround them. |

| 2 | Survive the Spadaro Ambush Don Galante is the first to fall, and Luca gets shot. After dealing with the Spadaro Mafiosos, you escort Don Torrisi out of the Tonnara as he tends to Luca. You meet Leo outside and make your way to Cesare waiting by the truck. Enemies persistently chase you, and you must make a final stand at a bar. Luca unfortunately perishes, and the family mourns his loss. |

| 2 | Reach the Royal Box Without Getting Caught As you sneak into the Opera House in Palermo, you must sneak through numerous police officers and reach the Royal Box. Baron Fontanella is unaware of your presence, and you easily take him out. |

| 3 | Chase Don Spadaro Don Spadaro catches you and flees. You must fight through Spadaro Mafiosos all the way back to the underground passages. Your pursuit comes to a close with a fight against Don Spadaro himself before bringing his ring to Don Torrisi, signifying Spadaro's end. |

| 1 | Return to the Vineyard Following the fall of Spadaro, the Torrisi family is now eliminating all those remaining who worked for Spadaro. Return to the vineyard and talk to Cesare regarding your plans with the three remaining Spadaro family heads. Along the way, you talk to Isabella about going to America and leaving this life behind. |

However, Tino and Cesare intercepts you and brings you back to the Collezolfo Sulfur Mine. Don Torrisi is furious with your betrayal and relentlessly tries to kill you. As the volcano nearby erupts, you fight Don Torrisi and finally kill him. In the end, Cesare betrays you. |

| 2 | Get to Enzo You play as Isabella, and must escape the house before Tino proceeds with the forced abortion. You go through the cellar, but Tino eventually finds you. By using the surroundings to hide and throwing bottles to distract him, you successfully kill Tino. Cesare arrives and gives Isabella two boat tickets, hinting at Enzo's fate. Isabella leaves and the scene cuts to her on a boat as she reads Enzo's final letter. |
