First met in Chapter 5. Sam Trapani, who played a major role in Mafia 1, is shown as a child of the Torrisi Family's underboss, Luca Trapani. |
Giuseppe Palminteri |
First met in Chapter 6. Giuseppe Palminteri, the fixer for the protagonist of Mafia 2, comes back to before his days in Empire Bay still working as a counterfeiter. |
Frank Vinci |
First met in Chapter 7. Frank Vinci, head of the Vinci Crime Family in Mafia 2, appears as a young and silent mafioso working for the Galante Crime Family. |
A few characters from previous Mafia games return in Mafia: The Old Country as younger versions of themselves. You really wouldn't recognize them unless you're familiar with the series since they play minor roles in the story.
There are optional interactions you can find throughout the game's story that are not required for progression but offers extra flavor for the player. This includes some interactions with Isabella, Cesare, and other NPCs.
If you're interested in finding these, take your time and approach characters throughout the story to see if they have anything to say about the current situation.

The title screen shows a common depiction of memento mori, an artistic symbol that acts to remind the viewer the inevitability of death.
You can see scattered objects on the title screen that represent different aspects of one's life and a skull to show the mortality and fleeting nature of it. This fits in well with the theme of the game as well as the life of a mafioso.

When you complete the game and finish Chapter 14, the title screen changes reflecting how the story ends.
The notes, newspapers, and other journal entries you find scattered throughout the game are not just collectibles but sometimes are references to previous Mafia games or world events (inside the game).

For example, one of the arrest records you find in the Guardia HQ during Chapter 6 shows Ennio Salieri, or Don Salieri, the main antagonist for the original Mafia game.

Newspapers show events and developments happening around the game's world such as the announcement of a New Pope, the Russo-Japanese War, new inventions, revolutions, and natural disasters among other things. These interesting tidbits really capture the game's setting and time period.
There are some moments in the game where you'll need a keen eye to notice or a curiosity for exploration. Look out for these sightings in the story or get back to them through either Chapter Replay or Explore Mode.
Mafia The Old Country Interactive Map

In Chapter 4, during your horse ride together with Isabella on the way to the ruins, you'll find two horsemen on the cliff observing you from afar. This foreshadows the kidnapping that happens later in the chapter.
Chapter 4: Il Barone Guide and Walkthrough

The name of Chapter 12 is based on an opera of the same name called La Forza del Destino. You can find the opera's script lying around during the chapter and the opera programme showing that La Forza del Destino is the opera performed during the course of the chapter.
The opera contains a scene where a father disowns his daughter for her involvement with a man, akin to the forbidden love between Enzo and Isabella under Don Torrisi's nose.

In the real world, La Forza del Destino has the reputation of being cursed due to some unfortunate incidents. As you'll find out as you play through the chapter, unfortunate incidents do indeed happen during the opera.

There are a bunch of hidden trophies and achievements you can go for in the game. While some of them can be obtained incidentally just by playing through the story, there are some that you really have to aim for.
Some of these are pretty hard to do the first time so go for them again by reloading checkpoint or through the Chapter Replay feature.

Some chapters have secret achievements tied to them where you have to do an optional objective or meet a certain challenge.
Walkthrough and List of All Chapters
Credits to a YouTube video by Manugames92, you can find a chair with a phonograph sitting on top in the middle of a structure in the ruins to the southeastern part of the map near L'Antica Riva. It doesn't really do anything and we don't know what it refers to in-game.
