Mafia: The Old Country ends in tragedy and betrayal for all the main characters. During the final chapter, Enzo and Isabella decide to escape to America with their unborn child, as they want to live away from their current lives. However, this is cut short as they are ambushed by Tino and Torrisi's mobsters. Despite having gained status, respect, and wealth throughout the game, Enzo doesn't get a happy ending by being involved with the Mafia and trying to turn away from his dirty life.

Enzo and Isabella are separated, with Enzo being sent to confront the Don back in the mines where he started, while Isabella is sent back to the villa. Enzo fights his way through Torrisi's mafiosos and Cesare, eventually leading to the final showdown. This ends with Don Torrisi's demise. Shortly after the duel, Cesare appears to reconcile and apologize to Enzo, but instead stabs him by surprise, ultimately killing Enzo.

Back in Villa Torrisi, Isabella attempts to escape a forced abortion and to get to Enzo. This proves to be difficult as Tino and the mafiosos have the place surrounded, all the while dealing with a volcanic eruption burning down most of the estate. Still, Isabella manages to sneak her way out, only to be caught again by Achille. This encounter ends with her killing him and taking his knife. As she gets farther away, she is spotted by Tino. However, she gains the resolve to kill him to finally escape.

As she is about to leave on horseback, she is stopped by Cesare. Cesare gives her a bloodied ticket for a boat ride and tells her to go. Realizing Enzo is gone, she grieves and sets off for America alone. The final scene shows Isabella on the boat reading Enzo's final letter and staring into the distance to start her new life.
There are no alternative endings or multiple endings in Mafia: The Old Country. The same ending sequence plays out regardless of whether you played it for the first time or from replaying the last chapter via Chapter Select.
