The Executioner Boss Guide
Boss of The Rot
The Executioner is a boss of The Rot. You'll meet her during the story, but only on your third encounter will you face her as a boss. To find her, choose the Axe symbol when picking rooms in The Rot.
How to Beat The Executioner

The Executioner slams the ground, causing fireballs to emit from the impact point and spawn in all directions. If this happens, stay far away from the spot, as the fireballs will fly outward and the hit area expands the farther they travel. |

The Executioner will also erect pillars of skeletons in a circle. A good indicator this is about to happen is the brown circle on the ground, so avoid the area if you see her attempting it. This attack becomes more frequent at low health, so only approach the Executioner from a safe distance. |

The Executioner sends her axe forward in one direction. This is easy to avoid, as you only need to move behind her and attack from there to deal damage. |

The Executioner commands its skeleton minions to dash toward you. You can counter this by dashing forward, or by defeating them first if there aren't too many. |
How to Get Hagar as a Follower
Pardon Hagar First
After the boss battle, you'll be given the option to either pardon the Executioner or execute her. To make her a follower, choose to pardon her. Her name will be revealed to be Hagar, and she will leave the boss room. After a while, she will appear in the town of Woolhaven.
Indoctrinate Hagar in Woolhaven
You'll have another option to pardon or spare the Executioner in Woolhaven. The ghosts of the Lambs are unhappy with Hagar's presence, since she is responsible for executing most of their kin.
Once you completely rebuild Woolhaven and unlock all quest boards, the residents will ask you to make a choice—whether to indoctrinate or kill Hagar. To get the Executioner as a follower, indoctrinate her from there.
As a follower, she has the unique Blind trait which causes her to bump into other followers. It doesn't have any negative or positive effects, and is just a cosmetic trait.

Swing of the Axe is a hidden achievement that you can get by beating The Executioner. This achievement is exclusive to the Woolhaven DLC, and by simply progressing, you'll earn it.
The Executioner Follower
As mentioned above, you can get The Executioner as a follower after defeating her. There is a process involved, but she's going to be one of the rare followers to have, as she's exclusive to the story.
