You can encounter this fight during the Hearts of Stone Main Quest: Scenes From a Marriage. This fight can be tricky since his attacks deal a lot of damage and he also has the ability to regenerate a lot of HP when he kills a ghost.
The best way to deal with The Caretaker is to attack him right after he uses a big attack like his shovel slam or his shovel charge. You can also use Northern Wind Bombs to freeze him in place and hit him with a fully charged Rend.
When his HP gets to a certain point, he will start summoning ghosts and attacking them. When this happens, it is ver important to kill the ghosts before he is able to attack him. Each ghost he kills will regenerate a lot of his HP.
You should use your Silver Sword and coat it with Relict oil to deal bonus damage to them. You can also prepare some Northern Wind Bombs if you prefer attacking him with Rend.
