Dropped by a Scarab found inside a cave with 4 hidden Albinaurics in Mohgwyn Palace. To reach the cave, head north from the Palace Approach Ledge-Road site of grace and hug the east wall until you find the cave with the Scarab.

Blood Tax is also the default Ash of War of Varre's Bouquet. To get this weapon, you will need to progress White Mask Varre's questline before invading and defeating him inside the Mohgwyn Dynasty Mausoleum in Mohgwyn Palace.
Varre's Bouquet Location and Stats
Blood Tax shares the same animations with Repeating Thrust, albeit with an added Blood Loss build up effect. It can be used to quickly inflict Bleed since the thrusts have good stagger damage, ensuring that anyone caught by the first hit will also be hit by the succeeding thrusts.
