The best ways to get Medals are through digging and fishing. With a Hoe in hand, you can dig mounds of dirt to get a regular and Bronze Medals guaranteed.
On the other hand, Fishing offers much rarer Medal types and treasures like Gold Medals and Treasure Chests, albeit on a lower chance.

Regular and Bronze Medals can be obtained by tilling mounds you see all over the map with a Hoe. Doing this method has a guaranteed chance of obtaining medals!
You can also till farmlands but with a low chance of obtaining Medals!

Fishing is always available versus digging when you want to actively search for Medals, but only at a certain chance. Try to upgrade your Fishing Rod to a higher tier to get more chances of obtaining Medals or other Treasures, which is indicated when you get a diamond-shaped marker when you get a bite!
An upgraded Silver Fishing Rod for example has a higher chance of obtaining Silver Medals on the watering hole near the shop of the Nature Sprites!

You can also make gifts out of your medals! Just keep in mind that not all characters will like this type of gift. Here are the characters that do like medal gifts:
