Mafia: The Old Country Overview

Mafia: The Old Country is an upcoming action-adventure game developed by Hangar 13 and published by 2K, that released on August 8, 2025, for PC, PlayStation 5, and Xbox Series X|S. Serving as a prequel to the original 2002 Mafia, it is the fourth main installment in the franchise and marks the series’ return after a nine-year hiatus.

Here, players assume the role of Enzo Favara, a young man who escapes a life of indentured servitude in Sicily’s sulfur mines and ultimately joins the Torrisi crime family in the fictional town of San Celeste, previously featured in Mafia II.

The game is a third-person, linear, narrative-driven experience that aligns more closely with the cinematic storytelling of the first two Mafia titles, as opposed to the non-linear approach of Mafia III. There will be no open world, but the game introduces new mechanics to the series like the "Respect" system, which influences interactions with NPCs, and a wound system that affects performance until treated.

Mafia: The Old Country Gameplay

Mafia: The Old Country emphasizes a linear, narrative-driven gameplay experience set in early 1900s Sicily. Departing from the open-world structure of Mafia III, it returns the series to the more focused progression of the first two installments. Players will navigate the game through the perspective of Enzo Favara, and can engage in both stealth and direct confrontations using period-appropriate weapons such as revolvers and stiletto blades. The game also incorporates a cover system with destructible environments.

The new Respect mechanic allows players to intimidate enemies to potentially avoid combat altogether. Additionally, if the player gets wounded, their character’s performance will be negatively affected until it gets treated.

Moreover, missions here are designed to reflect the player's choices and actions, with objectives that can change based on previous decisions. "Family Missions" require balancing the needs of the criminal organization with personal goals, where failure can lead to consequences such as betrayal or lost opportunities. Side activities include managing legitimate businesses as fronts for illegal operations, with a "Heat" level tracking police awareness and adapting their tactics accordingly. Environmental storytelling enriches exploration, offering discoverable backstories and secrets that can lead to unexpected mission opportunities or valuable resources .

Enzo Favara is the protagonist of Mafia: The Old Country. He begins life as a carusu, a child laborer, in Sicily's harsh sulfur mines, sold by his father to clear debts. His escape follows a deadly mine collapse and a violent encounter with Don Spadaro’s underlings. Rescued by Don Bernardo Torrisi, Enzo enters the Torrisi crime family, rising to become a soldato (soldier) under their tutelage.

As the patriarch of the Torrisi Crime Family, Bernardo Torrisi wields power and influence in early 1900s Sicily. He saves Enzo, mentors him, yet eventually becomes his adversary.

Isabella Torrisi

Isabella is Don Torrisi's daughter and becomes Enzo’s romantic interest. Yearning for a life beyond the family's criminal world, she dreams of escaping with Enzo to America, an aspiration that drives much of her storyline

La Famiglia Tells a Great Tale

Here at Game8, we gave Mafia: The Old Country an overall score of 86/100 for its great narrative pace, excellent writing and direction, and memorable characters. While it's undoubtedly a strong title, we noted that it's on the shorter side and that its gameplay loop remains fairly basic.

IGN gave it an 8/10, praising it as "an effective return to the linear and tightly story-driven format of the original Mafia and Mafia II." While the stealth mechanics and third-person cover shooting aren't groundbreaking, the game’s strong writing, stellar voice performances, and keen attention to historical detail make The Old Country captivating.

On Steam, the game holds a mostly positive rating of 3.5 stars at the time of writing. Players largely agree that it’s visually stunning, though relatively short in length.

Meanwhile, on Metacritic, it holds a generally favorable score of 75, with critics agreeing that while it doesn’t reinvent the wheel, it stands as a worthy narrative entry in the Mafia franchise.
